How long does a medical certificate last?

Medical Certificate Guide

Different medical certificates have different validity periods — and using an expired certificate, or one issued too early, is one of the most common reasons they are rejected. This guide covers the validity rules for every major certificate type, so you know exactly when to get yours and how long it will be accepted.

Quick Reference

Validity Periods at a Glance

Validity periods vary significantly between certificate types. Some are tied to specific travel dates, others to the duration of illness, and some have no fixed expiry at all. The table below summarises the standard rules — full details for each type are covered in the sections below.

Typical validity
Key rule

Sick note (fit note)

Must cover the departure date — submit to insurer within their claim window

Sports medical

Sick Notes

How Long Does a Sick Note Last?

There is no fixed maximum duration for a sick note in the UK. A GMC-registered doctor can issue a sick note for work covering any period they consider clinically appropriate — from a few days to several months. For long-term conditions, a single note may cover up to three months or longer.

When a sick note expires, a further note can be issued without necessarily requiring a new appointment, provided the condition is already documented. Private sick notes typically cover up to 14 days at a time, though longer periods can be issued where clinically supported.

No legal maximum duration — set by the doctor’s clinical judgement
Can be backdated if illness began before the assessment date, with supporting evidence
Can be extended with a further note when the original expires
NHS fit notes are free after 7 days — private certificates are required for shorter absences or early evidence requests

Fit to Fly

How Long Is a Fit to Fly Certificate Valid?

A fit to fly certificate is time-sensitive — most airlines require it to be issued within 24 to 72 hours before departure. A certificate issued more than a few days before travel may not be accepted as it does not reflect the passenger’s condition at the time of the flight.

The exact window varies between airlines. Some accept certificates up to seven days old; others specify 24 hours. Always confirm the required timeframe with your specific airline before booking a doctor’s appointment.

Most airlines require issuance within 24–72 hours of departure
For pregnancy, many airlines require a fresh certificate for each leg of a long journey
Airline MEDIF forms typically require advance submission of several working days
Always check your specific airline’s policy — requirements vary widely

Sports Medical

How Long Is a Sports Medical Certificate Valid?

Sports medical certificate validity varies significantly by sport, governing body, and event organiser. Unlike most other certificate types, there is no universal standard — each body sets its own requirements.

Most sports medical certificates are valid for between three and twelve months from the date of issue. Some events — particularly international marathons and triathlons — specify that the certificate must be dated within a set number of months before the event date. Always check the event registration requirements before requesting a certificate.

Typically valid for 3–12 months — check your specific event or governing body
Some events require the certificate to be issued within a specific window before the event date
Golf club buggy medical certificates are typically renewed annually
A new certificate is required for a new event season in most organised sports

Other Certificate Types

Validity Periods for Other Common Certificate Types

The following certificate types each have their own validity considerations. In most cases the certificate should be obtained as close to the point of use as practical — and submitted promptly once obtained.

Travel cancellation certificate

Must cover the specific departure date. There is no expiry on the certificate itself — what matters is that it is submitted to the insurer within their claim window, typically 28–31 days.

Gym cancellation letter

No fixed expiry. The certificate confirms the medical condition and its impact — submit it to the gym promptly as the cancellation will typically take effect from the date the gym receives and approves the evidence.

University / student sick note

Must cover the dates of the affected assessment. Submit within your institution’s extenuating circumstances deadline — typically five to ten working days from the assessment date.

Return to work certificate

Issued for the specific date of return. It is not valid for future return dates and cannot typically be used retrospectively. Obtain it on or shortly before the planned return date.

Stress leave certificate

Covers the period stated on the certificate — typically two to four weeks initially, with the option to renew. The same rules as sick notes apply regarding extension and backdating.

Allergy / exemption certificate

No universal validity period — depends on the purpose and the receiving organisation. Some may accept a certificate indefinitely; others require annual renewal. Confirm requirements before obtaining the certificate.

Expiry and Renewal

What Happens When a Certificate Expires?

Expiry rules depend on the certificate type. For sick notes and stress leave certificates, expiry means the covered period has ended — not that the document becomes invalid for the period it covered. For time-sensitive certificates like fit to fly, expiry means the certificate can no longer be used at all.

1

Sick note expired — still unwell

Contact your GP or a private medical service to obtain a further certificate. There is no need to start the process from scratch — a renewal note can be issued without a new full assessment in many cases, provided the condition is already documented. Inform your employer as soon as the existing note approaches its end date.

2

Fit to fly — missed the validity window

A fit to fly certificate obtained too early cannot simply be reused — a new certificate will be required closer to the travel date. Contact the issuing service to arrange a fresh assessment. Some services allow a rebooking at reduced cost if circumstances have not changed.

3

Sports medical — expired before the event

A new sports medical certificate is required. Most event organisers will not accept a certificate outside their stated validity window. Request the new certificate close enough to the event date to ensure it falls within the required period — but not so close that it cannot be processed before registration closes.

FAQ

Frequently Asked Questions

How long is a sick note valid for?

There is no fixed maximum. A sick note covers the period stated on it — which is determined by the issuing doctor based on clinical need. This can range from a few days to several months. When the note expires, a further note can be issued if you remain unwell. There is no legal limit on how many consecutive sick notes can be issued for an ongoing condition.
How long before a flight does a fit to fly certificate need to be issued?

Most airlines require a fit to fly certificate issued within 24 to 72 hours before departure, though requirements vary. Some accept up to seven days; others specify 24 hours. Always confirm the required timeframe directly with your airline before booking your assessment — getting this wrong is one of the most common reasons fit to fly certificates are rejected at check-in.
How long is a sports medical certificate valid?

It depends on the sport and governing body. Most sports medical certificates are valid for between three and twelve months. Some event organisers — particularly international races — specify that the certificate must be dated within a set number of months before the event. Always check the event’s registration requirements before obtaining your certificate.
Can I extend a sick note that has run out?

Yes. A further sick note can be issued to cover the period following expiry. Where the condition is already documented and has not changed significantly, this can often be done without a full new assessment. Contact your GP or a private medical service before the existing note expires where possible — this avoids a gap in your documentation that may cause administrative issues with your employer.
Does a medical certificate have an expiry date printed on it?

Most medical certificates state the dates they cover rather than an explicit expiry date. For sick notes, the end of the stated period is the effective expiry. For fit to fly certificates, the date of issue combined with the airline’s required window determines whether it is still valid. For certificates with no stated period — such as gym cancellation letters — there is no expiry, but prompt submission is always advisable.
