How long does a medical certificate last?
Different medical certificates have different validity periods — and using an expired certificate, or one issued too early, is one of the most common reasons they are rejected. This guide covers the validity rules for every major certificate type, so you know exactly when to get yours and how long it will be accepted.
Validity Periods at a Glance
Validity periods vary significantly between certificate types. Some are tied to specific travel dates, others to the duration of illness, and some have no fixed expiry at all. The table below summarises the standard rules — full details for each type are covered in the sections below.
How Long Does a Sick Note Last?
There is no fixed maximum duration for a sick note in the UK. A GMC-registered doctor can issue a sick note for work covering any period they consider clinically appropriate — from a few days to several months. For long-term conditions, a single note may cover up to three months or longer.
When a sick note expires, a further note can be issued without necessarily requiring a new appointment, provided the condition is already documented. Private sick notes typically cover up to 14 days at a time, though longer periods can be issued where clinically supported.
How Long Is a Fit to Fly Certificate Valid?
A fit to fly certificate is time-sensitive — most airlines require it to be issued within 24 to 72 hours before departure. A certificate issued more than a few days before travel may not be accepted as it does not reflect the passenger’s condition at the time of the flight.
The exact window varies between airlines. Some accept certificates up to seven days old; others specify 24 hours. Always confirm the required timeframe with your specific airline before booking a doctor’s appointment.
How Long Is a Sports Medical Certificate Valid?
Sports medical certificate validity varies significantly by sport, governing body, and event organiser. Unlike most other certificate types, there is no universal standard — each body sets its own requirements.
Most sports medical certificates are valid for between three and twelve months from the date of issue. Some events — particularly international marathons and triathlons — specify that the certificate must be dated within a set number of months before the event date. Always check the event registration requirements before requesting a certificate.
Validity Periods for Other Common Certificate Types
The following certificate types each have their own validity considerations. In most cases the certificate should be obtained as close to the point of use as practical — and submitted promptly once obtained.
Travel cancellation certificate
Must cover the specific departure date. There is no expiry on the certificate itself — what matters is that it is submitted to the insurer within their claim window, typically 28–31 days.
Gym cancellation letter
No fixed expiry. The certificate confirms the medical condition and its impact — submit it to the gym promptly as the cancellation will typically take effect from the date the gym receives and approves the evidence.
University / student sick note
Must cover the dates of the affected assessment. Submit within your institution’s extenuating circumstances deadline — typically five to ten working days from the assessment date.
Return to work certificate
Issued for the specific date of return. It is not valid for future return dates and cannot typically be used retrospectively. Obtain it on or shortly before the planned return date.
Stress leave certificate
Covers the period stated on the certificate — typically two to four weeks initially, with the option to renew. The same rules as sick notes apply regarding extension and backdating.
Allergy / exemption certificate
No universal validity period — depends on the purpose and the receiving organisation. Some may accept a certificate indefinitely; others require annual renewal. Confirm requirements before obtaining the certificate.
What Happens When a Certificate Expires?
Expiry rules depend on the certificate type. For sick notes and stress leave certificates, expiry means the covered period has ended — not that the document becomes invalid for the period it covered. For time-sensitive certificates like fit to fly, expiry means the certificate can no longer be used at all.
Sick note expired — still unwell
Contact your GP or a private medical service to obtain a further certificate. There is no need to start the process from scratch — a renewal note can be issued without a new full assessment in many cases, provided the condition is already documented. Inform your employer as soon as the existing note approaches its end date.
Fit to fly — missed the validity window
A fit to fly certificate obtained too early cannot simply be reused — a new certificate will be required closer to the travel date. Contact the issuing service to arrange a fresh assessment. Some services allow a rebooking at reduced cost if circumstances have not changed.
Sports medical — expired before the event
A new sports medical certificate is required. Most event organisers will not accept a certificate outside their stated validity window. Request the new certificate close enough to the event date to ensure it falls within the required period — but not so close that it cannot be processed before registration closes.
Clinical Review & Eligibility
All medical certificates issued through MedicalCert are reviewed individually by GMC-registered UK doctors. Every application is assessed on its own clinical merits — certificates are not automatically generated.
