What Is a Work Adjustment Medical Certificate?

A work adjustment medical certificate — also known as an adjustments to work duties letter — is a medical document from a GMC-registered doctor outlining recommended modifications to your work responsibilities based on your health condition. It communicates to your employer what changes are needed to allow you to work safely and effectively while recovering from illness or managing an ongoing condition.

What it does

Unlike a sick note, which certifies that you are unable to work, a work adjustment certificate confirms that you can continue working — subject to specific modifications. It bridges the gap between full sick leave and normal duties, enabling a graduated return or an adjusted working arrangement that protects your health.

This mirrors the “may be fit for work” assessment on an NHS Fit Note, but in a more detailed, standalone format appropriate for private documentation of specific adjustment recommendations.

Sick Note vs Work Adjustment Certificate — Which Do You Need?

🔧 Work Adjustment Certificate

  • You can work but need modifications to your duties or environment
  • Supporting a phased return after sick leave
  • Managing a chronic condition while remaining employed
  • Formally documenting a reasonable adjustment request
  • Returning early before a sick note expires with conditions

📋 Sick Note (Fit Note)

  • You are unable to work in any capacity
  • Absence for 8 or more consecutive calendar days
  • Certifying absence to access Statutory Sick Pay
  • Short-term acute illness preventing any attendance

If you are currently on stress leave or recovering from an illness and want to return to work in a modified capacity, a work adjustment certificate is the appropriate document. If you need to certify full absence, see our fit to work certificate for clearance when returning after sick leave.

Common Work Adjustments Covered

Our doctors can recommend a wide range of modifications where clinically appropriate. Adjustments are tailored to your specific condition and role. Certificates are suitable for low-risk, non-hazardous workplaces — high-risk environments (plant operators, scaffolding, road work) require specialist occupational health assessments.

Reduced working hoursPart-time or reduced hours during recovery, increasing progressively.

🏠

Remote or home workingWhere role permits, working from home to reduce commute fatigue or physical demands.

📋

Modified dutiesTemporary removal from tasks that aggravate your condition — manual handling, sustained standing, client-facing roles.

🪑

Ergonomic or physical modificationsRecommendations for adapted equipment, adjusted workstations, or physical access provisions.

🔄

Phased return to full dutiesA graduated plan from light duties back to full responsibilities.

🕐

Flexible start or finish timesAdjusted patterns to accommodate medication schedules, fatigue cycles, or treatment appointments.

🤝

Reduced targets or workloadTemporary reduction in performance targets or volume of work during recovery.

📅

Time off for medical appointmentsAcknowledgement of ongoing treatment needs and support for appointment attendance.

Conditions Commonly Supported by a Work Adjustment Certificate

Adjustments can be appropriate for both physical and mental health conditions — subject to clinical assessment. Common presentations include:

Back pain, musculoskeletal injuries
Stress, burnout, anxiety
Post-operative recovery
Chronic fatigue or long COVID
Depression or mental health conditions
Arthritis or joint conditions
Migraines or chronic headaches
Pregnancy-related health needs
Neurological or autoimmune conditions
Respiratory conditions (asthma, breathlessness)
Adjustment certificates are not appropriate for high-risk occupational environments requiring in-person assessment — plant operators, scaffolding workers, motorway road workers, traffic marshals, and similar roles. These require specialist occupational health evaluation. If in doubt, our doctors will advise you during clinical review.

How to Get a Work Adjustment Medical Certificate

1

Complete a medical questionnaire

Describe your health condition, how it affects your ability to perform your current duties, and what specific adjustments you are seeking. Include your job role and working environment — the more specific you are, the more tailored your certificate will be.

2

Upload supporting evidence

A brief video describing your symptoms and working situation, or supporting documentation such as a prescription, previous GP letter, or hospital discharge notes. Evidence of your condition helps the doctor make well-informed recommendations.

3

GMC-registered doctor reviews and issues your certificate

One of our experienced GPs reviews your submission the same day and — where clinically appropriate — issues a certificate specifying recommended work adjustments. The certificate will detail your condition, the adjustments recommended, and their expected duration.

4

Deliver to your employer and agree a plan

Your certificate is delivered by email same day or by 9AM next morning as a signed PDF. Present it to your employer or HR department as the basis for a return-to-work or ongoing adjustment discussion. Each certificate includes a unique reference number verifiable at verify@medicalcert.co.uk.

Work Adjustment Certificate FAQs

Employers are generally expected to accept medical certificates from GMC-registered practitioners and take the recommendations seriously. While acceptance of the specific adjustments is subject to the employer’s assessment of reasonableness (including operational feasibility and cost), a documented refusal to consider medical recommendations — particularly where a disability is involved — may constitute unlawful discrimination under the Equality Act 2010. We recommend discussing the certificate’s recommendations with HR and requesting any decisions in writing.
Yes. You can return to work at any time before a sick note expires if you feel well enough. A work adjustment certificate is useful in this situation — it documents your readiness to return in a modified capacity and informs your employer of the conditions under which your return is clinically supported. You do not need to see a doctor again solely to return to work.
A fit to work certificate confirms that you are medically cleared to return to full duties — often required by an employer before you resume work after sick leave. A work adjustment certificate, by contrast, confirms that you can work but with specific modifications to your role or environment. Some employees need both: a fit to work certificate to authorise their return, and an adjustment certificate to communicate the conditions of that return.
Yes. Medical evidence of how a health condition affects your ability to perform your role is a key part of any reasonable adjustment request under the Equality Act 2010. A certificate from a GMC-registered doctor provides that evidence and may strengthen your case if your employer disputes the need for adjustments. For formal disability accommodation documentation, see also our disability accommodation letter for housing and accommodation contexts.
Each certificate covers a defined period as recommended by the doctor based on your clinical presentation — typically linked to your expected recovery timeline or the ongoing nature of your condition. If your condition is chronic, adjustments may be recommended on an ongoing basis. If your needs change, you can request an updated certificate reflecting your current situation.
Work adjustment certificates from MedicalCert are suitable for low-risk, non-hazardous workplaces. For roles involving significant physical risk — plant operators, scaffold workers, motorway operatives, traffic marshals, or similar — your employer will likely require a specialist occupational health assessment in addition to or instead of a private GP letter. Our doctors will advise you if your role falls outside the scope of what can be addressed through a remote consultation.
You can describe what adjustments you believe would support your health and working capacity in your questionnaire. The doctor will consider these in light of your clinical presentation and recommend those they consider medically appropriate. Final clinical decisions rest with the reviewing doctor — the certificate reflects their professional assessment, not simply a patient request.
