What Is a Work Adjustment Medical Certificate?
A work adjustment medical certificate — also known as an adjustments to work duties letter — is a medical document from a GMC-registered doctor outlining recommended modifications to your work responsibilities based on your health condition. It communicates to your employer what changes are needed to allow you to work safely and effectively while recovering from illness or managing an ongoing condition.
What it does
Unlike a sick note, which certifies that you are unable to work, a work adjustment certificate confirms that you can continue working — subject to specific modifications. It bridges the gap between full sick leave and normal duties, enabling a graduated return or an adjusted working arrangement that protects your health.
This mirrors the “may be fit for work” assessment on an NHS Fit Note, but in a more detailed, standalone format appropriate for private documentation of specific adjustment recommendations.

The Legal Framework — Reasonable Adjustments and the Equality Act 2010
What the law requires
Under the Equality Act 2010, employers have a legal duty to make reasonable adjustments for employees with disabilities — defined as physical or mental impairments with a substantial, long-term adverse effect on normal day-to-day activities. This duty is triggered when the employer becomes aware (or should reasonably be aware) of a condition placing the employee at a substantial disadvantage.
A work adjustment certificate from a GMC-registered doctor provides the medical evidence that both supports your request and puts your employer on notice of your clinical needs. Where an employer fails to make reasonable adjustments despite medical evidence of need, this may constitute disability discrimination under the Act.
Not all conditions requiring adjustments meet the legal disability threshold — shorter-term or lower-impact conditions may still warrant adjustments as good practice. Many employers accommodate adjustment requests without requiring formal legal justification.
For formal disputes, ACAS offers free guidance and early conciliation services. Access to Work grants are also available through DWP for assessments and equipment costs linked to disability at work.
