Can You Be Signed Off Work with Stress in the UK?

Yes. Any GMC-registered doctor can sign you off work with stress if your symptoms are severe enough to impair your ability to do your job. You do not need a formal psychiatric diagnosis. The doctor assesses your current symptoms, including sleep disruption, persistent anxiety, inability to concentrate, emotional exhaustion, and physical symptoms such as headaches or chest tightness, and determines whether those symptoms prevent you from carrying out your normal work duties.

Being "signed off with stress" means a doctor has assessed your condition and issued a medical certificate confirming you are temporarily unfit for work due to stress, burnout, or mental exhaustion. This certificate protects your job, entitles you to Statutory Sick Pay, and gives your employer the documentation they need to record your absence properly.

Who can sign you off with stress?

Three types of doctor can issue a valid stress leave certificate in the UK. Your NHS GP can issue an NHS fit note (Med3) at no cost, though appointment wait times average two to three weeks. A private GP, either in-person or online, can issue a private medical certificate, typically same day. An online doctor service such as MedicalCert provides the same outcome through an online consultation reviewed by a GMC-registered doctor. All three routes produce a certificate with full legal weight, accepted by UK employers for absence records and SSP.

What does your employer see on a stress leave certificate?

The certificate states your name, the dates of absence, the name and GMC number of the issuing doctor, and that you are unfit for work. The clinical reason listed will typically be "work-related stress," "stress and anxiety," or a similar description agreed between you and the reviewing doctor. The certificate does not include details of your consultation, personal circumstances, or specific workplace grievances. Your employer receives only the information necessary to process your absence and Statutory Sick Pay. Under ACAS guidance, you are not required to explain the precise nature of your illness beyond what appears on the certificate.


What Is Work-Related Stress? The HSE Definition

"The adverse reaction people have to excessive pressures or other types of demand placed on them at work."

This is the formal definition used by the Health and Safety Executive (HSE), the UK's workplace health regulator. It draws a clear line between normal workplace pressure, which can be motivating, and stress, which is a harmful reaction that occurs when pressure exceeds a person's capacity to cope. Under this definition, work-related stress is always damaging and is a recognised occupational health hazard.

The HSE's Management Standards identify six primary areas of work that, if poorly managed, lead to work-related stress: demands (workload, work patterns), control (autonomy over how work is done), support (from managers and colleagues), relationships (conflict, bullying), role (clarity of responsibilities), and change (how organisational change is communicated). When Do You Need a Stress Leave Certificate?

The UK rules for stress absence follow the same threshold as all other sick leave.

0-7 calendar days

Self-certification only. Inform your employer you are unwell. Stress is a valid reason. A formal doctor's certificate is not required for this period. You are still entitled to Statutory Sick Pay (SSP) if eligible. All calendar days count, including weekends and bank holidays.

8+ calendar days

A fit note or private stress leave certificate is required. Your employer can legally request this. MedicalCert provides same-day private stress leave certificates with no GP appointment required. Submit online and receive your certificate same day or by 9AM the next morning.

Some employers require a doctor's note for shorter absences if you have a pattern of frequent short-term stress-related leave, or where stress is identified as a recurring occupational health concern. For Employers: Managing an Employee's Stress-Related Absence

If an employee has been signed off with stress, employers have specific obligations under UK health and safety and employment law. The following guidance aligns with ACAS recommendations on managing work-related stress absence.

What employers should do when an employee is off with stress

1.
Accept the medical certificate. Employers must accept fit notes and private medical certificates from GMC-registered doctors. You cannot reject a valid stress leave certificate or demand a specific format.
2.
Agree a reasonable level of contact. ACAS recommends agreeing with the employee how often and by what method you will stay in touch during their absence. The goal is to prevent isolation without being overbearing.
3.
Conduct a stress risk assessment. Under the Management of Health and Safety at Work Regulations 1999, you are required to identify and act on workplace stress risks. If an employee has been signed off with stress, this should prompt a review of workload, management practices, and working conditions in that role.
4.
Hold a return-to-work meeting. When the employee is ready to return, meet to discuss any work updates, outstanding adjustments, and what support they need. Mind's Wellness Action Plan is a practical tool for structuring this conversation.
5.
Consider reasonable adjustments. If the employee's stress has led to a condition that meets the Equality Act 2010 definition of disability, you have a legal duty to make reasonable adjustments. Even where the condition does not meet this threshold, ACAS recommends considering adjustments as good practice.

Do not penalise the absence. Legitimate stress-related absence with proper medical certification should not count against an employee in disciplinary proceedings, absence management triggers, or performance reviews.

How Long Can You Be Signed Off Work with Stress?

There is no legal maximum period for stress leave in the UK. The duration is determined entirely by clinical assessment. Your GP or private doctor evaluates your symptoms, functioning, and recovery trajectory to determine the appropriate period.

Stress Leave in Specific Workplace Situations

The general rules above apply in all cases, but certain workplace situations raise specific questions. Each scenario below provides a standalone answer.

Can I be signed off with stress while on probation?

Yes. Probationary employees have the same right to sick leave and Statutory Sick Pay as permanent staff from day one of employment. Your employer cannot terminate your probation solely because you take stress-related absence supported by a valid medical certificate. However, some employment contracts include specific attendance requirements during probation. If your employer raises concerns, check your contract and contact ACAS for free guidance on your rights.

Can I take stress leave while working my notice period?

Yes. If you are clinically unfit for work due to stress during your notice period, you are entitled to take sick leave and receive SSP in the normal way. Your notice period continues to run while you are off sick. You do not need to extend your notice to make up for the time absent, unless your employment contract states otherwise. A stress leave certificate from MedicalCert is valid during a notice period in the same way as during any other employment period.

What if I cannot get a GP appointment for stress?

NHS GP wait times in the UK average two to three weeks for a routine appointment. If you need a stress leave certificate and cannot see your GP quickly enough, a private online service such as MedicalCert can issue a same-day certificate following an online consultation with a GMC-registered doctor. The certificate carries the same legal weight as one issued by your NHS GP. You do not need to be registered with the private service as your regular GP.

Am I eligible for stress leave on a zero-hours or part-time contract?

Yes. Following the April 2026 SSP reforms, the previous minimum earnings threshold (Lower Earnings Limit) for Statutory Sick Pay has been abolished. Part-time workers, zero-hours contract workers, and lower earners who previously did not qualify for SSP now do. SSP is payable from day one of absence at £123.25 per week or 80% of average weekly earnings, whichever is lower. What happens when my stress leave certificate expires?

If you remain unfit for work when your certificate's coverage period ends, you need a new certificate to continue your authorised absence. Without a valid certificate covering the dates, your employer may treat the absence as unauthorised. Each MedicalCert stress leave certificate covers up to 14 days. You can request an extension by submitting a new online consultation. The reviewing GP will reassess your current symptoms and issue a further certificate if clinically appropriate. There is no limit on the number of consecutive certificates you can obtain, provided the clinical basis for each is established at the time of review.

A well-managed return reduces the risk of relapse. Your stress leave certificate can include recommendations for workplace adjustments to support your return.

Common return-to-work adjustments after stress leave

Your employer has a duty under the Equality Act 2010 and their general duty of care to consider reasonable adjustments. These are not automatic, but a medical certificate recommending them significantly strengthens your position. Common adjustments include:

Temporary reduction in workload or responsibility
Working from home for a transitional period
Change of line management or team
Adjusted start and finish times

If your employer cannot accommodate the recommended adjustments, the certificate may effectively be treated as "not fit for work" for a further period. Consider also a work adjustment certificate if you need formal documentation of recommended modifications upon your return.

Do you need a sick note for a phased return to work after stress?

A phased return to work means gradually increasing your hours or responsibilities over a set period, typically two to four weeks, rather than returning to full duties immediately. Your stress leave certificate can include a recommendation for a phased return, which your employer should consider as part of their duty of care and, where applicable, their Equality Act 2010 obligations.

If you need more detailed documentation for a structured phased return, such as specific recommended hours, duties to avoid, or a weekly progression plan, a separate phased return to work letter provides the level of detail your employer and HR department can use to build a formal plan. This is particularly useful in larger organisations where occupational health, HR, and line management all need a clear framework.
