What Is a Stress Leave Certificate?
A stress leave certificate is a medical document issued by a GMC-registered doctor confirming that work-related stress, burnout, or mental exhaustion has rendered you temporarily unfit for work. It provides your employer with documented medical evidence for your absence, supports your Statutory Sick Pay claim, and records that the time off was medically certified rather than unauthorised.
In the UK, stress leave is not a separate legal category. It falls under standard sick leave. The process and documentation are the same as for any other medical absence. Can You Be Signed Off Work with Stress in the UK?
Yes. Any GMC-registered doctor can sign you off work with stress if your symptoms are severe enough to impair your ability to do your job. You do not need a formal psychiatric diagnosis. The doctor assesses your current symptoms, including sleep disruption, persistent anxiety, inability to concentrate, emotional exhaustion, and physical symptoms such as headaches or chest tightness, and determines whether those symptoms prevent you from carrying out your normal work duties.
Being "signed off with stress" means a doctor has assessed your condition and issued a medical certificate confirming you are temporarily unfit for work due to stress, burnout, or mental exhaustion. The certificate gives your employer the medical evidence they need to record the absence as authorised sick leave and to process Statutory Sick Pay. It does not by itself guarantee your continued employment, though dismissing an employee for legitimate certified sickness absence carries a real risk of an unfair dismissal claim, and separate protections may apply under the Equality Act 2010.
Who can sign you off with stress?
Three types of doctor can issue a stress leave certificate in the UK. Your NHS GP can issue an NHS fit note (Med3) at no cost, though appointment wait times can run to several weeks. A private GP, either in-person or online, can issue a private medical certificate, typically same day. An online doctor service such as MedicalCert provides the same outcome through an online consultation reviewed by a GMC-registered doctor. An NHS fit note must be accepted by your employer as evidence of incapacity. A private certificate is accepted by most UK employers, but acceptance depends on the employer's own absence policy.
What does your employer see on a stress leave certificate?
The certificate states your name, the dates of absence, the name and GMC number of the issuing doctor, and that you are unfit for work. The clinical reason listed will typically be "work-related stress", "stress and anxiety", or a similar description agreed between you and the reviewing doctor. The certificate does not include details of your consultation, personal circumstances, or specific workplace grievances. Your employer receives only the information necessary to process your absence and Statutory Sick Pay. Under ACAS guidance, you are not required to explain the precise nature of your illness beyond what appears on the certificate.
What Is Work-Related Stress? The HSE Definition

"The adverse reaction people have to excessive pressures or other types of demand placed on them at work."
This is the formal definition used by the Health and Safety Executive (HSE), the UK's workplace health regulator. It draws a clear line between normal workplace pressure, which can be motivating, and stress, which is a harmful reaction that occurs when pressure exceeds a person's capacity to cope. Under this definition, work-related stress is always damaging and is a recognised occupational health hazard.

Work-related stress in the UK: the scale of the problem
HSE Labour Force Survey data confirms that work-related stress is the single largest cause of occupational ill health in the UK. The figures below are the most recent published estimates, covering 2024/25 and released in November 2025.

When Do You Need a Stress Leave Certificate?
The UK rules for stress absence follow the same threshold as all other sick leave.

Self-certification only. Inform your employer you are unwell. Stress is a valid reason. A formal doctor's certificate is not required for this period. You are still entitled to Statutory Sick Pay (SSP) if eligible. All calendar days count, including weekends and bank holidays.
8+ calendar days

Stress leave is one of several mental health related reasons for time off work. The right document depends on what is actually keeping you from work. A stress leave certificate is the correct choice when workplace stress, burnout, or mental exhaustion is the primary cause. Your Legal Rights on Stress Leave in the UK
Key employment law protections
Equality Act 2010: If your stress has led to a diagnosable condition such as an anxiety disorder or depression with a substantial and long-term effect on daily activities, it may meet the legal definition of a disability, giving you additional protection and entitlement to reasonable adjustments from your employer. Health and Safety at Work Act 1974: Employers have a legal duty to protect the health, safety, and welfare of employees. This includes mental health and work-related stress. Failure to address known occupational stress hazards may expose an employer to civil liability.
Management of Health and Safety at Work Regulations 1999: These regulations require employers to conduct a "suitable and sufficient assessment" of the risks to employee health, including risks from work-related stress. This is the legal basis for the HSE's stress risk assessment framework.
Statutory Sick Pay (SSP): Following the April 2026 reforms, SSP is payable from day one of absence. The previous three waiting days have been abolished. The current rate is £123.25 per week, or 80% of average weekly earnings, whichever is lower. Stress-related absence qualifies in the same way as physical illness. Employment Rights Act 1996: Dismissing an employee solely for taking legitimate, properly certified sick leave, including stress-related absence, carries a significant risk of an unfair dismissal claim. Whether a dismissal is unfair depends on the circumstances and is ultimately a matter for an employment tribunal.
Confidentiality: Your employer cannot contact your doctor without your explicit written consent. Beyond what appears on the certificate itself, you are not obliged to disclose the details of your condition. How Long Can You Be Signed Off Work with Stress?
There is no legal maximum period for stress leave in the UK. The duration is determined entirely by clinical assessment. Your GP or private doctor evaluates your symptoms, functioning, and recovery trajectory to determine the appropriate period.

Each MedicalCert stress leave certificate covers up to 14 days of absence. If you remain unfit to work when your certificate expires, you can request an extension. The reviewing GP will reassess your case based on your current symptoms and situation.
Following the April 2026 SSP reforms, Statutory Sick Pay is payable from day one of absence at £123.25 per week for up to 28 weeks. After 28 weeks of certified absence, SSP ends, though company sick pay policies may provide longer coverage depending on your employment contract.
Stress Leave in Specific Workplace Situations

Can I be signed off with stress while on probation?
Yes. Probationary employees have the same right to sick leave and Statutory Sick Pay as permanent staff from day one of employment. Employees with under two years' service have more limited unfair dismissal protection, though dismissal connected to a disability under the Equality Act 2010 is unlawful from day one. Can I take stress leave while working my notice period?
Yes. If you are clinically unfit for work due to stress during your notice period, you are entitled to take sick leave and receive SSP in the normal way. Your notice period continues to run while you are off sick. You do not need to extend your notice to make up for the time absent, unless your employment contract states otherwise. What if I cannot get a GP appointment for stress?
NHS GP appointment waits vary widely by practice and can run to several weeks for a routine appointment. If you need a stress leave certificate and cannot see your GP quickly enough, a private online service such as MedicalCert can issue a same-day certificate following an online consultation with a GMC-registered doctor. Most UK employers accept a private certificate as evidence, though this is at their discretion under their absence policy. You do not need to be registered with the private service as your regular GP.
Am I eligible for stress leave on a zero-hours or part-time contract?
Yes. Following the April 2026 SSP reforms, the previous minimum earnings threshold (Lower Earnings Limit) for Statutory Sick Pay has been abolished. Part-time workers, zero-hours contract workers, and lower earners who previously did not qualify for SSP now do. SSP is payable from day one of absence at £123.25 per week or 80% of average weekly earnings, whichever is lower. What happens when my stress leave certificate expires?
If you remain unfit for work when your certificate's coverage period ends, you need a new certificate to continue your authorised absence. Without medical evidence covering the dates, your employer may treat the absence as unauthorised. Each MedicalCert stress leave certificate covers up to 14 days. You can request an extension by submitting a new online consultation. The reviewing GP will reassess your current symptoms and issue a further certificate if clinically appropriate. There is no limit on the number of consecutive certificates you can obtain, provided the clinical basis for each is established at the time of review.
Returning to Work After Stress Leave
A well-managed return reduces the risk of relapse. Your stress leave certificate can include recommendations for workplace adjustments to support your return.
Where your condition meets the Equality Act 2010 definition of disability, your employer has a duty to make reasonable adjustments. Outside that, adjustments fall under their general duty of care and ACAS good practice rather than a legal obligation. A medical certificate recommending specific adjustments strengthens your position in that conversation. Common adjustments include:

Do you need a sick note for a phased return to work after stress?
A phased return to work means gradually increasing your hours or responsibilities over a set period, typically two to four weeks, rather than returning to full duties immediately. Your stress leave certificate can include a recommendation for a phased return, which your employer should consider as part of their duty of care and, where applicable, their Equality Act 2010 obligations.
If you need more detailed documentation for a structured phased return, such as specific recommended hours, duties to avoid, or a weekly progression plan, a separate phased return to work letter provides the level of detail your employer and HR department can use to build a formal plan. This is particularly useful in larger organisations where occupational health, HR, and line management all need a clear framework.
