Custom Medical Certificate

A custom certificate is individually drafted by a GP for your specific requirements, whereas a standard certificate uses a proven template for common situations. Standard certificates cover scenarios like sick notes, fit-to-fly letters, and sports medicals with fixed wording that has established acceptance. Custom certificates are tailored to unusual employer forms, legal language, non-standard health situations, or any requirement that a template cannot address. Both are signed by a GMC-registered doctor after individual clinical review. Choose a standard certificate if your situation matches an existing category, and custom if it does not.

Yes. Form completion is one of the most common uses of the custom certificate service. Upload the form along with your medical evidence and the reviewing GP will complete the sections that require clinical assessment or signature, where clinically appropriate. If further clarification is needed, the GP will contact you directly before proceeding. This applies to forms from employers, sporting bodies, educational institutions, insurers, and other organisations requiring a GP signature.

You need a custom certificate if your situation does not match one of our standard certificate categories, or if the receiving party has specified requirements that a templated certificate cannot meet. Common indicators include: you have been sent a form for a GP to complete and sign; your employer has requested specific wording; your health condition is not covered by our standard certificate range; or you are unsure whether a standard certificate will satisfy the requirement. When in doubt, request a custom letter. The reviewing GP will assess clinical appropriateness during review.

Custom certificates are signed by a GMC-registered doctor after individual clinical review, with the doctor's registration number and a QR verification code included. Final acceptance depends on your employer's own policy. Some employers accept any doctor-signed letter; others have specific requirements. If you know what your employer needs, include those requirements in the questionnaire and the GP will address them where the clinical evidence supports doing so. If you are uncertain, check your employer's policy before submitting.

There is no fixed evidence requirement, but more documentation enables a more detailed and clinically robust certificate. Common evidence types include: GP or hospital letters, discharge summaries, test results, prescriptions, diagnosis letters, clinic notes, and any relevant medical documentation. If you have limited evidence, the reviewing GP may contact you for additional information before completing the letter. Yes. One round of edits is included at no extra cost. We send you the draft certificate for review before it is signed. If you would like changes, for example additional clinical detail or different emphasis, let us know and we will make the edits. The doctor certifies their own clinical opinion, so edits adjust how that opinion is expressed rather than what it says, and a statement the evidence does not support will not be added. Once you approve the draft, the GP signs it and the final PDF is delivered to you.

Most custom certificates are reviewed and issued the same day you submit them. Most are delivered by 9AM the next morning. Each request is reviewed individually by a GP, not processed automatically. Actual turnaround depends on the complexity of the request and the time of submission, but requests are prioritised and not held in a queue. The draft review stage does not add delay for straightforward requests.

You receive a full refund if the reviewing GP determines there is insufficient clinical basis to issue the certificate. No questions, no conditions, no delay. Clinical integrity is not compromised to complete a sale. This means you can submit your request without financial risk. If the GP cannot help, you will be informed clearly and the refund will be issued immediately.

Custom certificates can be used abroad, but acceptance is decided by the employer, insurer, authority or overseas body receiving them, and requirements vary by country. Check what the recipient needs before ordering. Some countries require a specific form, an in-person examination, or the document to be legalised with a Hague Apostille from the FCDO Legalisation Office. MedicalCert does not arrange legalisation. You apply for it yourself at gov.uk/get-document-legalised after you receive your certificate.

Every certificate includes a unique reference number and a QR code. Third parties can scan the QR code to access the verification record, or contact us at verify@medicalcert.co.uk with the reference number for written confirmation. Verification is free and available to any receiving party. They can also verify the signing GP's registration and qualification directly through the GMC register at gmc-uk.org.
