Extreme Sports Medical Certificate
Skydiving, mountaineering, and off road racing operators often require a signed medical certificate before allowing participation. Our registered UK GPs assess your health online and issue the clearance you need without the delay of a face to face appointment.
Each certificate follows an individual clinical assessment by a licensed UK doctor, giving operators a credible document confirming you are medically cleared to take part.

Extreme Sports Medical Certificate — Adventure Activity Fitness Clearance
An extreme sports medical certificate — a fitness to participate extreme sports clearance document — is a signed document from a GMC-registered GP confirming that a participant has no known medical contraindication to taking part in a high-adrenaline activity. Adventure activity operators, challenge event organisers, and overseas activity providers require this documentation before allowing participation — both to protect participants and to meet their own insurance and liability obligations.MedicalCert issues adventure sports medical certificates following individual GP review — covering recreational extreme and adventure activities where a private GP assessment is appropriate. No appointment needed. Complete a secure health questionnaire, describe your activity and any health conditions, upload relevant evidence, and receive a signed adventure sports fitness clearance certificate same day or by 9AM next morning. Full refund if a certificate cannot be issued on clinical grounds.
What This Certificate Covers — and What Requires In-Person Assessment
MedicalCert’s GPs can provide extreme sports medical certificates for most recreational and adventure activities where a GP fitness opinion is the standard requirement. Some activities require specialist in-person examination — knowing the difference matters before applying:✔ Covered by this service
- ✔Recreational and charity skydiving — the British Parachute Association requires a GP-completed medical form; a private GP certificate fulfils this requirement
- ✔Mountaineering and high-altitude trekking where the operator requires a general fitness certificate from a GP
- ✔White-water rafting, kayaking, and open-water swimming events with operator medical requirements
- ✔Bungee jumping — most UK operators require a GP medical certificate or health declaration for participation
- ✔Obstacle course races (OCR), ultra-marathons, and multi-day endurance events
- ✔Off-road vehicle racing, quad biking, and motorsport events at non-professional level where a GP certificate is requested
- ✔Via ferrata, rock climbing courses, and adventure activities at outdoor centres
⚠️ Requires specialist in-person assessment
- ✗Professional or competition skydiving under British Parachute Association competitive licences — requires a BPA-approved doctor
- ✗Aviation sports including paragliding competition, powered paraglider, and hang-gliding at licence level — requires a CAA medical
- ✗Professional motorsport and FIA racing licences — require dedicated in-person FIA medical assessment
- ✗Scuba and technical diving — see our Scuba Diving Medical Certificate page for recreational diving
- ✗Olympic and elite-level competitive sports requiring specialist sports physician clearance
Why Adventure Operators Require Medical Clearance
Activity providers — from bungee jump operators to charity skydive organisations — require medical clearance for two reasons: participant safety and insurance liability. Most commercial adventure activity operators carry public liability insurance that requires documented fitness screening before high-risk participation. Without it, they expose themselves to significant legal and financial risk if a participant suffers a health event during the activity. An extreme sports fitness certificate from a GMC-registered GP provides operators with the evidence they need. It confirms that a qualified doctor has reviewed the participant’s health and identified no known contraindications to the planned activity.
Why your NHS GP may be unable to help
NHS GPs are not covered by the clinical negligence scheme that applies to NHS practice for extreme sports fitness assessments — meaning many NHS surgeries decline to issue these certificates as a matter of policy. This is not a reflection on your health; it is an NHS indemnity boundary. MedicalCert’s private GPs operate under private medical indemnity that covers adventure activity fitness assessments, which is why this service exists.
Activities Covered — Common Requests
Charity skydiving certificate
A charity skydiving certificate — also called a skydiving medical certificate for recreational jumps — is one of the most common requests. Most UK charity skydive operators use a BPA-style medical questionnaire requiring GP sign-off. Our GP assesses your health history and, where appropriate, confirms fitness for a tandem or AFF recreational skydive. Age limits and cardiac history are the primary areas assessed.
Mountaineering medical certificate
For guided mountaineering expeditions, high-altitude trekking (including Kilimanjaro, Everest Base Camp, and similar), and alpine climbing, operators often require a mountaineering medical certificate confirming no contraindication to sustained high-altitude exertion. Cardiovascular and respiratory fitness are the primary clinical considerations.
Bungee jumping medical certificate
Most UK bungee jump operators require participants to complete a health declaration or obtain a doctor’s clearance where health conditions are flagged. A bungee jumping medical certificate confirms no cardiovascular, neurological, or musculoskeletal contraindication to the brief but intense forces involved.
White-water rafting medical certificate
A white water rafting medical certificate is required by many commercial operators, particularly for Grade 4 and 5 rivers. Swimming fitness, cardiovascular capacity, and the absence of conditions that could cause sudden incapacitation underwater are the key clinical areas reviewed.
Obstacle course race (OCR) certificate
Events including Tough Mudder, Spartan Race, and similar obstacle course racing events often require medical clearance for participants with flagged conditions. An obstacle course medical certificate confirms fitness for sustained cardiovascular and musculoskeletal exertion over a multi-obstacle course.
Off-road racing and motorsport
Non-professional off-road vehicle events, quad biking challenges, and recreational motorsport events increasingly require a GP fitness clearance letter. An extreme sports fitness certificate covering the relevant activity is accepted by most non-competitive event organisers as satisfactory medical evidence.
